Dear Patient



We at The Elms feel it is time we moved on with the demands of increasing number of patients, CQC requirements and patient directives to relocate to a new, modern, patient friendly, CQC compliant, larger and more purpose built premise.


We have been considering and assessing very carefully over a period of time about a possible relocation of The Elms to a new more modernised property.


This initiative is purely a result of our constant effort in improving patient care and providing opportunity to widen our horizons to provide further services, e.g. minor surgery, implants, ground floor access to the elderly, wheel chair bound and disabled patients, teaching facilities for medical students and GP registrars etc.


We also wanted to stay as close as possible but at the same time to move to a building which is more patients friendly, has ease of access and not far from the present location, is more purpose designed and has provision for ample car parking facility and prevent the patient crossing over the busy road without proper pedestrian crossing.


We are extremely lucky enough to find such a property close enough to the existing premise which seems to fulfil the criteria.

Would like to thank all our patients for your continued support throughout the past year with all the changes that have occurred during the move.

For those of you who do not know yet here is a gentle reminder that The Elms Practice is now on line.  You can order your repeat prescriptions, cancel appointments, change address, give feedback update your records, request text reminders and book a telephone consultation appointment on line.  You can also fill in a questionnaire with your opinion of the Practice.

During our flu clinics we held a coffee morning in aid of the Macmillan nurses and we raised a fantastic £530.  Thank you to all who donated.

ASTHMA CLINIC— Asthma checks are given by our Practice Nurses Beryl and Cathy. 

If you have any concerns please make an appointment at the desk.

DIABETIC CLINIC—Diabetic clinic is run in conjunction with the Hospital.  We see diabetic patients in our Nurse clinics.

TRAVEL HEALTH— If you are planning a trip abroad please make a telephone appointment with our Nurse who will assess your Travel needs and arrange an appointment for vaccinations or refer you to the appropriate clinics for private vaccination and arrange for a private prescription for anti-malaria tablets.

DIETITIAN– An appointment can be made with the dietitian at the surgery for advice on weight reduction cholesterol reduction, diabetic advice and general dietary advice.  Patients are usually referred by the GP or Practice Nurse.                                        

WELL PERSON CLINIC—Our Practice Nurses are very experienced and fully qualified and are able to deal with all aspects of health and well being





We are happy to accept requests for repeat prescriptions by post, fax or in person and also on our website www.theelmspractice.nhs.uk.  It is preferable for you to use the pre printed slip which is attached to your prescription. Please tick the items you require.  Please delete any item that is no longer required. Using the pre printed slip will ensure that you receive your correct medication and will reduce the number of queries which have to be dealt with.

We need 48 hours to process your prescription but you can make the request up to 7 days in advance.   This is especially helpful to the practice around Bank Holiday times when we are very busy. If you have any problems with your prescription please ask for Jenny, our prescription administrator.  She can be contacted on telephone number  01695  588710 option 1 every day 8.30am 3.30pm
