We also have a new Practice Manager who started with us on the 1st February 2016 her name is Yvonne Keogh. 



Dr Krishnamurthy is the lead at the practice and is our Principal GP.  



If you have any concerns/suggestions or complaints our usual procedure applies and you can contact Yvonne Keogh our Practice Manager either by telephone or in writing, Yvonne is more than happy to speak to you or meet with you if you prefer.


For those of you who do not know yet here is a gentle reminder that The Elms Practice is now on line.  You can order your repeat prescriptions, cancel appointments, change address, give feedback update your records, request text reminders and book a telephone consultation appointment on line.  You can also fill in a questionnaire with your opinion of the Practice.

ASTHMA CLINIC— Asthma checks are given by our Practice Nurses Cathy and Paula.


If you have any concerns please make an appointment at the desk.

CLINIC—Diabetic clinic is run in conjunction with the Hospital.  We see diabetic patients in our Nurse clinics, also Copd, Chd.

TRAVEL HEALTH— If you are planning a trip abroad please make a telephone appointment with our Nurse who will assess your Travel needs and arrange an appointment for vaccinations or refer you to the appropriate clinics for private vaccination and arrange for a private prescription for anti-malaria tablets.

DIETITIAN– An appointment can be made with the dietitian at the surgery for advice on weight reduction cholesterol reduction, diabetic advice and general dietary advice.  Patients are usually referred by the GP or Practice Nurse.                                        

WELL PERSON CLINIC—Our Practice Nurses are very experienced and fully qualified and are able to deal with all aspects of health and well being





We are happy to accept requests for repeat prescriptions by post, fax or in person and also on Patient access.


It is preferable for you to use the pre printed slip which is attached to your prescription.


We now have a phone line for prescription orders from 11am until 1pm only option 2 on our main number.


Please tick the items you require.  Please delete any item that is no longer required. Using the pre printed slip will ensure that you receive your correct medication and will reduce the number of queries which have to be dealt with.


We need 48 hours to process your prescription but you can make the request up to 7 days in advance. 


This is especially helpful to the practice around Bank Holiday times when we are very busy.
